Knowledge Gap:
Chronic diseases are among the most costly and, in part, preventable health problems in the United States. Hispanic/Latino communities and Marshallese communities are disproportionately affected by chronic diseases and the impact can be felt at the individual, family and community level. There is a phenomenal growth rate in Northwest Arkansas among these two communities. In order to adequately meet their health care needs, we must begin to learn, understand and appreciate the culture and diversity within and among the groups relative to their health care behaviors and beliefs.

Target Audience
The target audience includes all healthcare professionals such as physicians, nurses, pharmacists and allied health professionals who would like to learn about Chronic Disease Management: Trends within the Hispanic/Latino and Marshallese Communities and how it pertains to their respective profession.

Objectives

  1. Define chronic disease and chronic disease management
  2. Articulate the importance of chronic disease management
  3. Describe leading chronic disease conditions relative to focus communities
  4. Recognize health beliefs, behaviors, and practices relative to focus communities
  5. Describe techniques useful for working with focus communities
  6. Explore the inclusion of the 5 A’s Model of Behavior Change for Self-Management

 

Biographical Info
Dr. Thessalenuere Bernard is an educator, researcher, administrator, and speaker with over a decade of experience spanning across academic and corporate environments. Dr. Bernard holds a Ph.D. in Human Development and Family Studies as well as an undergraduate and graduate degree in Sociology. She has managed and conducted research projects, provided extension and outreach services to various communities and diverse populations, as well as taught classes in family studies, diversity/multicultural perspectives, human development, housing and the social environments, and professional development.  She has a passion for diversity and inclusion, healthy homes, and healthy communities.
